In musculoskeletal examination, especially in the context of Mechanical Diagnosis and Therapy (MDT), directional preference and Modic changes have traditionally guided clinical reasoning and diagnostic pathways. Although each is informative, their absence, paradoxically, may provide even more clarity regarding screening accuracy.
Patients presenting with no directional preference (DP)—i.e., no symptomatic or mechanical response to one direction of movement—tend to be outside the general MDT classification paradigms. Likewise, the lack of Modic changes on imaging (which represent vertebral endplate and bone marrow changes) can lower the suspicion for particular structural or inflammatory spinal pathologies. When both of these findings are present together—no Modic change and no DP—they can narrow diagnostic uncertainty and direct clinicians to more targeted differential assessment by refining the screening process.
This review discusses how integrating mechanical and imaging results increases screening reliability, especially in complicated or uncertain presentations of low back pain. By incorporating the lack of directional preference and Modic changes into clinical judgment, practitioners are able to enhance decision making, prevent unwanted interventions, and more accurately detect cases for further evaluation or alternative treatment approaches.
In a health care setting that requires both effectiveness and precision, knowing what is not there can be as important as knowing what is there. Awareness of these negative signs allows clinicians to perform with increased diagnostic accuracy and assurance—improving both patient and resource utilization.
