Competition athletes commonly present with hip and groin complaints, but identifying the precise location can be a daunting task. In contrast to most other joints, the hip has few sensitive clinical tests that can accurately "rule in" pathology, making differential diagnosis both necessary and subtle. A broad range of conditions can present similarly to one another, from intra-articular issues such as labral damage, femoroacetabular impingement (FAI), and early osteoarthritis, to extra-articular sources such as adductor tendinopathy, iliopsoas pain, and stress fractures. In certain instances, referred pain from the sacroiliac joint or lumbar spine can confuse the issue. The nuances of history, mechanism of injury, and clinical presentation must be understood to enable proper assessment. This article discusses the essential differentials that clinicians must weigh when assessing athletes with hip and groin pain, identifies the shortcomings of existing examination methods, and underscores a systematic, comprehensive method of diagnosis and treatment.
